        <p>Mental disorders constitute a major and growing contributor to the global burden of disease, accounting for a substantial proportion of disability adjusted life years (DALYs) and economic loss. Despite this, mental health remains insufficiently integrated into mainstream public health systems, resulting in fragmented care, limited preventive strategies, and persistent inequities in access. Psychiatry has traditionally focused on individualized diagnosis and treatment, whereas public health emphasizes prevention, early intervention, and population level strategies. The lack of integration between these domains has constrained the effectiveness of mental health responses, particularly in low- and middle-income countries (LMICs) where resources are scarce and treatment gaps exceed 70%.</p>
        <p>This narrative review synthesizes evidence from WHO mental health reports, epidemiological datasets, and peer reviewed literature published between 2010 and 2024 to examine the rationale, barriers, and opportunities for integrating public health principles into psychiatric practice. Key themes include the epidemiological imperative for integration, structural and systemic barriers, successful models of community based and digital mental health care, and the central role of prevention in modern mental health systems.</p>
        <p>Findings indicate that integrated public mental health approaches offer scalable, cost effective solutions such as community mental health programs, digital interventions, task shifting models, and stigma reduction strategies. However, persistent barriers—including fragmented funding, workforce shortages, biomedical dominance, weak surveillance systems, and political underprioritization—continue to impede progress. Addressing these challenges requires a paradigm shift toward proactive, prevention oriented, population focused mental health systems supported by robust policy frameworks, cross sector collaboration, and sustained investment.</p>
        <p>Strengthening public mental health infrastructure and embedding mental health within primary care, schools, workplaces, and community organizations will be essential to mitigating the global mental health crisis. Integrated approaches can reduce treatment gaps, improve equity, enhance resilience, and deliver long term economic and societal benefits.</p>

      <title>INTRODUCTION</title>
      <p>Mental health has long occupied a paradoxical position within global health systems: widely recognized as essential to overall wellbeing, yet consistently underprioritized in policy, funding, and service delivery. Although mental disorders contribute significantly to disability adjusted life years (DALYs), mental health has historically been treated as an adjunct rather than a core component of public health. As noted in the manuscript, “The separation of psychiatric services from broader health systems has resulted in fragmented care, inequitable access, and limited preventive strategies.” This separation is not merely structural; it reflects deeper conceptual divides between individualized biomedical care and population level public health approaches.</p>
      <p>Public health systems evolved primarily to address infectious diseases, sanitation, maternal and child health, and later chronic disease prevention. Mental health, by contrast, developed within medical and institutional frameworks focused on diagnosis, containment, and individualized treatment. As a result, mental health services often operate in parallel to mainstream health systems, with limited integration into primary care, community health, or preventive programs. This fragmentation has contributed to persistent treatment gaps, inefficient resource allocation, and a lack of coordinated responses to emerging psychosocial risks [1-2].</p>
      <p>The consequences of this divide are particularly evident in low- and middle-income countries (LMICs), where mental health budgets frequently represent less than 1% of national health expenditure. In these settings, specialist psychiatric services are scarce, and primary care providers often lack training in mental health assessment or management. Without integration into public health systems, mental health remains peripheral, reactive, and insufficiently equipped to address population level needs. The global mental health crisis therefore reflects not only rising prevalence but also structural misalignment between mental health needs and the systems designed to address them.</p>
      <sec>
        <title>Global Pressures Intensifying Mental Health Needs</title>
        <p>The global mental health landscape is being reshaped by a constellation of pressures that extend far beyond traditional clinical determinants. Demographic transitions, socioeconomic instability, technological change, and environmental disruption are collectively intensifying mental health risks across populations [3-4]. These pressures highlight the limitations of specialist centric psychiatric models and underscore the need for public health integration.</p>
        <p>Rapid urbanization has altered social structures, increasing exposure to overcrowding, pollution, social isolation, and economic precarity. Urban environments often amplify stressors such as housing insecurity, unemployment, and reduced social cohesion. Migration whether voluntary or forced introduces additional risks, including trauma, cultural displacement, and barriers to accessing care.</p>
        <p>Socioeconomic inequality remains one of the strongest predictors of mental health outcomes. Individuals living in poverty face higher rates of depression, anxiety, and substance use disorders, driven by chronic stress, limited access to services, and exposure to violence or instability [5-8]. As the manuscript notes, “Socioeconomic inequality is strongly correlated with higher rates of depression, anxiety, and substance use disorders.” Public health systems are uniquely positioned to address these upstream determinants through targeted interventions, social protection policies, and community-based resilience programs.</p>
        <p>Climate change represents an emerging and increasingly urgent mental health threat. Extreme weather events, displacement, food insecurity, and eco anxiety contribute to rising rates of trauma, stress, and psychological distress. These environmental pressures require coordinated, multisectoral responses that extend beyond the remit of traditional psychiatric care.</p>
        <p>Digital transformation has introduced new psychosocial risks, including cyberbullying, misinformation, digital addiction, and the mental health effects of constant connectivity. Children and adolescents are particularly vulnerable to these digital stressors. Public health frameworks offer tools for population level digital literacy campaigns, regulatory strategies, and early detection systems that can mitigate these risks.</p>
        <p>Collectively, these global pressures demand mental health systems capable of addressing both individual and population level needs. Public health integration provides the infrastructure necessary to respond to these complex, interconnected challenges.</p>
      </sec>
      <sec>
        <title>Historical Context</title>
        <p>The historical evolution of mental health systems provides essential context for understanding current structural barriers. For much of the 19th and early 20th centuries, psychiatric care was dominated by institutional models that prioritized containment, custodial care, and social control. Large asylums were designed to manage severe mental illness rather than prevent it, and public health systems focused almost exclusively on infectious diseases and sanitation [9-11].</p>
        <p>The mid-20th century witnessed a shift toward deinstitutionalization, driven by human rights movements, psychopharmacological advances, and recognition of the harms associated with long term institutionalization. While deinstitutionalization aimed to transition care to community settings, many countries dismantled psychiatric hospitals without establishing adequate community infrastructure. As the manuscript notes, “many countries dismantled psychiatric hospitals without building adequate community infrastructure, resulting in persistent service gaps.” This left large populations without access to appropriate care and placed additional strain on families, primary care systems, and social services.</p>
        <p>During the same period, public health systems expanded their capacity for chronic disease prevention, maternal and child health, and health promotion. However, mental health remained peripheral, often excluded from national health strategies, surveillance systems, and funding mechanisms. The absence of mental health from public health agendas reinforced the perception that mental disorders were primarily clinical issues requiring specialist intervention rather than population level challenges requiring multisectoral action.</p>
        <p>The rise of global mental health as a field in the early 21st century marked a turning point. Landmark publications, including the Lancet Commission on Global Mental Health, emphasized the need for integrated, multisectoral approaches and highlighted the economic and societal costs of untreated mental illness [12-15]. International frameworks such as the WHO Comprehensive Mental Health Action Plan further underscored the importance of embedding mental health within public health systems.</p>
        <p>Despite these advances, historical legacies continue to shape contemporary mental health systems. Biomedical dominance, specialist centric training, fragmented funding, and limited public health competencies within psychiatric education remain significant barriers. Understanding this historical context is essential for designing integrated systems that address both clinical and population level needs.</p>
        <p>&#13;
  <bold>Consequences of Siloed Systems</bold>&#13;
</p>
        <p>Individuals with mild to moderate mental health conditions often fall through the cracks due to limited primary care training and resources. Specialist services remain overwhelmed by severe cases, leaving little capacity for prevention or early intervention. Public health approaches can rebalance systems by embedding mental health promotion and early detection within primary care, schools, workplaces, and community organizations.</p>
        <p>Globalization and technological change have introduced new mental health risks, including cyberbullying, misinformation, and digital overexposure. Public health frameworks are better equipped to address these emerging risks through population level digital literacy campaigns and regulatory strategies [16-20].</p>
        <p>Mental disorders including depression, anxiety, psychosis, and substance use disorders are among the leading causes of disability worldwide. Despite this burden, psychiatry remains predominantly reactive, focusing on diagnosis and treatment after illness onset. Public health prioritizes prevention, early intervention, and population level strategies.</p>
        <p>The manuscript emphasizes that “the siloed nature of these disciplines limits the effectiveness of mental health responses, particularly in LMICs where resources are scarce.” This siloing is particularly problematic given the scale of unmet need.</p>

  <bold>Treatment Gaps and Global Inequities</bold>&#13;
</p>
        <p>The WHO estimates that one in eight people worldwide live with a mental disorder, yet more than 70% receive no treatment in many LMICs. Even in high income countries, fragmentation, stigma, and workforce shortages impede access to care. The treatment gap is not only a clinical issue but a structural one, reflecting insufficient integration of mental health into primary care, public health systems, and community-based services.</p>

        <p>Depression and anxiety alone cost the global economy more than USD 1 trillion annually in lost productivity. </p>
        <p>Mental health intersects with chronic disease management, maternal health, adolescent health, and aging. Untreated depression worsens outcomes in diabetes and cardiovascular disease. Anxiety increases obstetric risks. Adolescents with mental health challenges face higher risks of substance use and school dropout. Integrated systems are therefore essential.</p>

    </sec>
    <sec>
      <title>METHODOLOGY</title>
      <p>A narrative review approach was used to synthesize evidence across epidemiology, service delivery, prevention, digital health, and policy. Sources included:</p>

Traditional psychiatric models, which emphasize individualized diagnosis and treatment, often struggle to account for the dynamic interactions between social determinants, community structures, and health system capacity. Public health frameworks, by contrast, are designed to address complexity through population level surveillance, multisectoral coordination, and preventive strategies.</p>
        <p>Systems thinking enables policymakers and clinicians to identify leverage points within mental health ecosystems such as primary care integration, school-based interventions, and digital platforms that can produce large scale improvements in access and outcomes [21-24]. This approach also supports the development of stepped care models that allocate resources efficiently by matching intervention intensity to clinical need.</p>
        <p>&#13;
  <bold>Integration with Primary Care and Chronic Disease Management</bold>&#13;
</p>
        <p>Primary care remains the most accessible point of contact for most populations, yet mental health is often insufficiently embedded within routine primary care workflows. Integrating mental health into primary care improves early detection, reduces stigma, and enhances continuity of care. Collaborative care models where primary care providers work alongside mental health specialists have demonstrated strong evidence for improving outcomes in depression, anxiety, and comorbid chronic diseases.</p>
        <p>Given the bidirectional relationship between mental and physical health, integrated care pathways are essential. Depression worsens glycaemic control in diabetes; anxiety increases cardiovascular risk; and substance use disorders complicate HIV and hepatitis management. Public health systems can support these pathways through population level screening, health promotion campaigns, and chronic disease registries that incorporate mental health indicators [25-30].</p>

  <bold>Community Engagement and Social Capital</bold>&#13;
</p>
        <p>Community based approaches are central to public mental health. Social capital defined as the networks, relationships, and norms that enable collective action plays a protective role against mental illness. Strengthening community structures through peer support networks, local mental health champions, and culturally adapted interventions enhances resilience and reduces reliance on specialist services.</p>
        <p>Community engagement also improves acceptability and uptake of interventions, particularly in LMICs where cultural beliefs and stigma may limit access to formal psychiatric care. Public health systems can mobilize community resources to support task shifting initiatives, school based programs, and workplace mental health strategies.</p>
        <p>&#13;
  <bold>Digital Public Mental Health Infrastructure</bold>&#13;
</p>
        <p>Digital mental health is no longer an adjunct but a core component of modern mental health systems. Public health agencies increasingly rely on digital tools for surveillance, early detection, triage, and intervention delivery. Mobile apps, telepsychiatry, AI driven risk prediction, and digital self management tools expand access, particularly in underserved regions.</p>
        <p>Digital platforms also enable population level interventions such as digital literacy campaigns, stigma reduction initiatives, and remote monitoring systems. When integrated with public health infrastructure, digital tools can reduce costs, improve continuity of care, and support real time decision making.</p>
        <p>&#13;
  <bold>Governance, Financing, and Policy Alignment</bold>&#13;
</p>
        <p>Effective integration requires governance structures that align mental health with national public health priorities. Financing mechanisms must shift from specialist centric models toward prevention oriented, community-based systems. Policy alignment across health, education, social care, and technology sectors ensures coherent implementation and sustained impact.</p>
        <p>Public mental health governance also benefits from robust surveillance systems that provide actionable data for planning, resource allocation, and evaluation.</p>
      </sec>
    </sec>
    <sec>
      <title>RECOMMENDATIONS</title>

      <title>CONCLUSION</title>
      <p>The global mental health crisis represents one of the most pressing public health challenges of the 21st century. Rising prevalence, widening inequalities, demographic transitions, and emerging psychosocial stressors have exposed the limitations of traditional psychiatric models that rely heavily on specialist led, clinic based care. As highlighted throughout this review, psychiatry’s individualized, reactive orientation while essential for the treatment of severe mental disorders cannot alone address the scale, complexity, and upstream determinants of contemporary mental health needs.</p>
      <p>Public health frameworks offer complementary strengths that are indispensable for modern mental health systems. These include population level surveillance, prevention, early detection, health promotion, and the ability to address social determinants such as poverty, inequality, climate related stressors, and digital harms. Integrating these principles into psychiatric practice is not merely beneficial; it is essential for building resilient, equitable, and sustainable mental health systems.</p>
      <p>Successful models from the UK, Chile, and digital mental health innovations demonstrate that integration is both feasible and impactful when supported by strong policy frameworks, community engagement, and investment in workforce development. Task shifting approaches have expanded access in LMICs, while digital tools have improved continuity of care and reduced costs. Prevention particularly in youth mental health offers high return on investment and long term societal benefits.</p>
      <p>However, achieving meaningful integration requires addressing persistent structural barriers. Fragmented funding, workforce shortages, biomedical dominance, weak data systems, and political underprioritization continue to impede progress. Overcoming these challenges demands coordinated action across health, education, social care, technology, and policy sectors. Governments must embed mental health within national public health strategies, allocate sustainable funding, strengthen surveillance systems, and invest in culturally adapted, community-based interventions.</p>
      <p>A paradigm shift is needed from reactive, illness focused care to proactive, prevention oriented, population level mental health systems. Such a shift will reduce treatment gaps, improve equity, enhance resilience, and strengthen community engagement. Ultimately, integrating public health and psychiatry offers the most viable pathway to addressing the global mental health crisis and ensuring that mental health is recognized, resourced, and delivered as a fundamental component of public health.</p>
