New NSW pathology hub to launch in Westmead Hospital Precinct

The Australian government is set to establish a new $321m (A$492m) statewide pathology hub at Westmead Hospital Precinct in New South Wales.

The hub will include a new build for the Institute of Clinical Pathology and Medical Research.

It will also include an upgrade to the high-security PC4 biocontainment laboratory, which is used to test, isolate and research high-risk infectious diseases that have the potential to be a major infection threat.

There are over 100,000 clinical and scientific investigations undertaken each day in NSW, with 70 per cent of medical decisions relying on pathology investigations and results.

The pathological classification of nonspecific interstitial pneumonia (NSIP) with organizing pneumonia (OP) overlap (NSIP/OP overlap) remains complex due to overlapping pathological features, and its heterogeneity is not well understood. We retrospectively analysed adult patients with interstitial lung disease (ILD) diagnosed with NSIP/OP overlap via surgical lung biopsy. Patients were pathologically subclustered using an unbiased clustering method, and clinical, radiological, and prognostic differences were examined. Among 38 patients, two pathological clusters were identified: Cluster 1, characterized by fibrotic changes with mild inflammation, and Cluster 2, exhibiting intense inflammation with fibrosis. While both clusters initially responded well to treatment, Cluster 2 demonstrated progressive ILD deterioration and a higher frequency of pulmonary fibrosis. Cluster 2 was also associated with hypoxia, reduced pulmonary function, elevated erythrocyte sedimentation rate, and greater consolidation on chest computed tomography. Based on these findings, we have identified NSIP/OP overlap is a heterogeneous and progressive disease and pathological findings at diagnosis significantly influence both initial ILD severity and long-term prognosis. Augmented reality microscopy to bridge trust between AI and pathologists

Diagnostic certainty is the cornerstone of modern medicine and critical for maximal treatment benefit. When evaluating biomarker expression by immunohistochemistry (IHC), however, pathologists are hindered by complex scoring methodologies, unique positivity cut-offs and subjective staining interpretation. Artificial intelligence (AI) can potentially eliminate diagnostic uncertainty, especially when AI “trustworthiness” is proven by expert pathologists in the context of real-world clinical practice. Building on an IHC foundation model, we employed pathologists-in-the-loop finetuning to produce a programmed cell death ligand 1 (PD-L1) CPS AI Model. We devised a multi-head augmented reality microscope (ARM) system overlayed with the PD-L1 CPS AI Model to assess interobserver variability and gauge the pathologists’ trust in AI model outputs. Using difficult to interpret regions on gastroesophageal biopsies, we show that AI-assistance improved case agreement between any 2 pathologists by 14% (agreement on 77% vs 91%) and among 11 pathologists by 26% (agreement on 43% vs 69%). At a clinical cutoff of PD-L1 CPS5, the number of cases diagnosed as positive by all 11 pathologists increased by 31%. Our findings underscore the benefits of fully engaging pathologists as active participants in the development and deployment of IHC AI models and frame the roadmap for trustworthy AI as a bridge to increased adoption in routine pathology practice.
